The Eastney Practice is planning to merge with Trafalgar Medical Group Practice (Based in Osborne Road and Eastney Health Centre) in April 2018

Together we will form a bigger, stronger Practice better able to provide services you need for the forseeable future.

Please read more about this in our merger FAQ's available from the Reception and our website from the links below or under the further information section on the right hand side. Please let us know your comments by writing to the Practice Manager at either surgery.

Patients can now register with The EASTNEY PRACTICE to access our clinical system online services.  Book or cancel appointments. Order your repeat prescriptions.  Access your summary care record.  Ask the receptionist for details.

Patients of the THE EASTNEY PRACTICE only. You can now make appointments, order prescriptions and view some of your clinical records on line.  We will be holding 2 online registration mornings on Saturday 1st April and Saturday 15th April 2017 between 8am – 10 am . If you would like to come along and register, we will ask you to complete a form at reception. Please bring photo ID with you. Please note it will not be possible for you to collect prescriptions, book appointments or speak to a Doctor on these 2 Saturday mornings.

THE PRACTICE WAS RATED AS GOOD OVERALL FOLLOWING OUR CQC INSPECTION IN APRIL.  WE HAVE BEEN RATED AS OUTSTANDING FOR THE CARE WE GIVE OUR VULNERABLE PATIENTS.  PLEASE CLICK ON THE LINK TO YOUR RIGHT TO VIEW THE REPORT.

Named Accountable GP for all patients

All our patients now have a named accountable GP.  The named accountable GP is responsible for patients’ overall care at the practice.  If you would like to know who that GP is please contact the surgery.   If you have a preference as to which GP you would like as your named GP, the practice will make every effort to accommodate this request. 


 

This surgery will be closed on the following afternoons from 1pm so our doctors, nurses and staff can attend training events.

The Eastney Practice

The Doctors are proud to offer the highest standard of patient-centred healthcare.  We run many clinics for the management of chronic diseases such as asthma and diabetes and offer a wide variety of other medical services including antenatal and postnatal care, childhood vaccinations and well-person check-ups.

In addition to everything you need to know about the practice you will also find a wealth of health-related information in the menu on the right hand side.


 The practice  offers an extended hours service for pre-bookable  appointments only. 

           *Doctors: Alternate Saturday mornings:  8-10 am

           *Practice Nurses (not vaccinations) Tuesday and Thursday early mornings:  7.00- 8.00 am  

           * Please note the Health Centre is not open for general enquiries or prescription collection during the extended hours service.   If you have an emergency please follow the normal procedures by telephoning 111 for the Out of Hours service 

  

To ensure you receive the right care we may share information about you and your care with other health professionals. We will only use or pass on identifiable information about you with other health professionals who are involved in the direct provision of your care. They will ask your permission to see your information when they first see you. We will not disclose your identifiable information to anyone else without your permission unless in exceptional circumstances (i.e. life or death situations), or where the law requires it.

The NHS currently uses your information in an anonymous and safe way to:

  • protect the health of the public
  • help us anticipate, plan and provide care
  • audit and monitor the quality of services provided
  • Information held about you may be used to help protect the health of the public and to help us manage the NHS. Information may be used for clinical audit to monitor the quality of the service provided and design new services that fit our local patients health needs.  Your information may be used by NHS Portsmouth Clinical Commissioning Group in order to ensure appropriate planning of current and future NHS commissioned services, this data will contain no identifiable information.  NHS Portsmouth Clinical Commissioning Group  will only ever use information that has been anonymised and all identifying information removed.

     

By registering with this practice you are consenting to your information being shared.

However, you can ask for your information not to be shared outside of the practice. Please complete the information sharing opt-out form if you do not wish your information to be shared. Please look under the link DATA SHARING. If you decide to opt out it will not affect your entitlement to care. However, it may result in the delivery of your care being less efficient as clinicians will not see your full medical history. If you have any concerns about how your information is shared or held, please contact the Practice Manager.
